This thyroid was signed out as "PTC", with a note (features suggestive of a follicular variant). Resection confirmed the dx. NOW, in the era of NIFTP, the best dx would be "Suspicious for Malignancy" (S-PTC), with a note "The possibility of NIFTP cannot be entirely excluded".

4. When I ask PGY1s why they think a cell is malignant they often say "prominent nucleoli". A much better criterion is irregularity of nuclear contour. Benign nuclei tend to be symmetric, malignant nuclei are often not. This gives rise to the "4 quadrant rule" #sblppgy1
