This adult male was crushed by a half ton object causing these various injuries - he was HD unstable despite proper initial resuscitation and with a pelvic binder applied - algorithms are ok but the optimal interventions/timings are usually best when customized for each patient

Was difficult to bring the plate to the bone due to the deformity. Most difficult was the rot deformity seen on axial ct. Used short screws to avoid going thru PFJ. Though plate position distally is not ideal.The # appears healing at 4 mo but with 2, 3.5 screws broken 2/2🧵

Third tubular plates fashioned into 1 hook and 2 spring-hook implants - the surgeon either forgot/didn’t know/ignored that these implants are weak in bending strength and require a supporting well contoured recon plate applied over the unstable wall fragments to function properly
