Isotretinoin - commonly known as Accutane - is a vitamin A derivative that’s proven highly effective at permanently clearing severe acne. Yet, after four decades of use, scientists still aren’t exactly sure why it works. Over time, isotretinoin has raised growing concerns for causing a wide array of side effects - some persisting even after discontinuation in what’s now called “Post Accutane Syndrome” (PAS). Side effects range from relatively mild (hair loss, dry skin) to far more troubling - most alarmingly, links to psychosis. In a notable 2015 case, lawyers argued that a 15-year-old on isotretinoin experienced a psychotic episode leading to homicide. Shockingly, that wasn’t an isolated incident. Beyond isolated trials, multiple anecdotal reports tie isotretinoin to lasting psychiatric disturbances. Equally disturbing is the persistence of these symptoms. Just as isotretinoin can resolve acne permanently, for some unlucky patients its side effects also become permanent. A groundbreaking 2005 study using brain imaging of patients treated with the acne drug found an enormous 21% decrease in brain activity in a region of the prefrontal cortex.
