I don’t understand this. Would we say “colon polyp not cancer don’t treat”? Or what about prophylactic mastectomy for BRCA? Treat if reduction in risk > harm. Nothing to do with what we call the lesion.

In the prebiopsy setting, risk-adapted and reasonable complementation of mpMRI with PSMA-PET in select cases will likely provide the best clinical net benefit. Let's not get back to biopsy all men with elevated PSA ... biopsy avoidance is one of the main achievements of MRI.
