📌 Post-operative Follow-up: Within 3 months, her HbA1c improved to 6.8%. Steroid tapering was initiated and closely monitored.
Post-operative Findings: On post-op day 1, cortisol levels were 1.5 mcg/dl, indicating adrenal insufficiency, which was managed with hydrocortisone.
📌 Management: The patient underwent successful transsphenoidal pituitary surgery
📌 Diagnosis: Her "type 2 diabetes" was secondary to Cushing’s disease, caused by an ACTH-secreting pituitary adenoma.
📌 Imaging: MRI of the pituitary: A 9 mm hypointense lesion was found in the right pituitary, strongly suggestive of an ACTH-secreting adenoma.
📌 The Next Step: Screening Tests We decided to screen for cushings Overnight 1 mg dexamethasone suppression test: No suppression, cortisol = 11.4 mcg/dl. 24-hour urinary free cortisol: Elevated at 345 mcg/24 hr (normal <50). ACTH: 86 pg/mL (elevated).
📌 Initial Thoughts: At first glance, this appeared to be a classic case of T2DM, likely worsened by weight gain. However, subtle features like facial puffiness and muscle weakness didn’t fully align with what we'd expect from uncontrolled T2DM. This led us to dig deeper.
📌 Key History: Gradual weight gain (15 kg in 3 years) Increasing fatigue and muscle weakness over the last year. Complaints of facial fullness and a sensation of puffiness around her neck. Irregular menstrual cycles with longer gaps. No family history of diabetes.
Despite her efforts, her blood sugars remain high, and she's gained significant weight over the past few years. It appeared to be a straightforward case of type 2 diabetes, but the situation wasn’t as simple as it seemed

Learning Points Most common causes of pancreatitis in India are alcohol and FCPD. However, it is worthwhile looking for secondary causes of the same (hypertriglyceridemia, hypercalcemia, hyperparathyroidism) especially in patients with recurrent episodes and/or bony pains
It was three weeks before she could sit without support again. She was discharged on calcium supplements and physiotherapy. Within a year, she was back to her old self and off all medications.
