I have scrolled through the "experts" telling us why primary care is so hard and how to fix it. The "fix" starts with greater investment (real money, not incentives) and then with getting rid of all the administrative trash that clutters the days of primary care physicians.

10/ The only other example of a primary care practice chain that isn't risk-based generating good margin is One Medical, and they did it by piggybacking commercial rates for hightech employers on hospital fee for service rates-not exactly in line w Walmart x.com/farzad_md/stat…
In general, primary care services are slow to scale, and aren't going to have huge margins no matter how tech-enabled... unless they move to taking risk on total cost of care. Their current clientele are not ideal for risk (too healthy).
15/ I don't want to minimize their challenges, they live in the world of shitty primary care FFS reimbursement and at the mercy of whatever contracts they are handed by payers. but you know what they can also amazingly well-with a little help? perform in risk-based contracts
14/ I had dinner last night w 3 rural Texas docs (town pop 2,000 to 5,000) who are delivering fantastic primary care at homes, in office, hospital and international missions 4 family med graduates among their children. competing softball teams. a combined 80 years in practice
13/ They work hard, but they are less likely to be burnt out than their employed brethren they may well be practicing in an 80 year old practice their children are more likely to go into medicine, and to take over their legacy they are scrappy - and more likely to be happy

12/ We work with hundreds of rural practices at Aledade, and they are amazing!! They still round on patients in the hospital, they are county health officers, and softball team doctors. they serve their church, and their community. they are often entrepreneurs on the side too

7/ If the business model then isn't taking risk on total cost of care, then their meaning of "value-based care" becomes more like "value meal" - cheap primary care But fee for service primary care is already a low margin business - and getting worse. doing it cheaper is HARD
6/ But they partnered w UHG instead. at one level I get it, UHG know how to run an MA plan. But, for once, Walmart is on the wrong side of a huge information assymetry in that partnership, and probably on the wrong side of the economics as well x.com/farzad_md/stat…

2/ The first thing I have to acknowledge is to rule out "execution" They aren't perfect (their Athena and Epic EMR travails show that) but Walmart knows how to execute, and they won't scale something until they've figured out how to make it profitable. They couldn't
1/ When Walmart enters any business you can expect that they will leverage their massive scale to get better economics, create value for customers- and drive out local mom and pop competitors Thats what many assumed would happen w primary care clinics but it didn't why not?
When the argument is always "No margin, no mission" there is no limiting factor on what profit-seeking behavior tax-exempt hospitals will engage in. Open a private equity subsidiary? 340B at a clinic in Bevery Hills? Garnish wages for unpaid bills? "No margin, no mission!"
