#orthotwitter do you guys still test for mrsa prior to tka and tha? I do total shoulders so I don’t. Do you treat +patients? @NaanDerthaal @jointdocShields @generalorthomd @OrthoThompson
@patelorthodoc @NaanDerthaal @jointdocShields @OrthoThompson We test all patients. I treat total joints with topical mupirocin.
@patelorthodoc @NaanDerthaal @generalorthomd @OrthoThompson We do - positives get mupirocin and vanc + ancef
@patelorthodoc @jointdocShields @generalorthomd @OrthoThompson We threat everyone with mupirocin swabs.
@patelorthodoc @NaanDerthaal @jointdocShields @generalorthomd @OrthoThompson I test all patients and treat with mupirocin if positive. The benefit for determining MRSA colonization is that I give vancomycin preoperatively with cefazolin.
@patelorthodoc @NaanDerthaal @jointdocShields @generalorthomd @OrthoThompson Believe it’s been shown cost effective to just treat everyone with nasal muporicin and test no one @Dr_Stambough
@patelorthodoc @NaanDerthaal @jointdocShields @generalorthomd @OrthoThompson Are people then giving vanc to pinnings and short nails?
@patelorthodoc @NaanDerthaal @jointdocShields @generalorthomd @OrthoThompson No test. Universal decolonization with PI nasal swab. All patients get screening questionnaire in clinic and high risk get vanco and ancef at surgery. For hip fractures I do screen them when they hit the door per our hip fx protocol.
@patelorthodoc @NaanDerthaal @jointdocShields @generalorthomd @OrthoThompson Yes test all and treat + with @NozinPro preop. If can’t get test than treat anyway. Vanco/ Ancef preop prophylaxis in (+)
