Understanding plasma osmolality: 2×Na⁺ accounts for sodium and its anions. Glucose and BUN are converted from mg/dL to mmol/L which makes them to divide with 18 and 2.8 respectively. K⁺ is excluded due to its low plasma concentration & minimal osmotic effect.
