I tried repeatedly to "Invite" my anaesthetist and surgeon to have a conversation with me about dying (and loss of capacity). They were extremely uncomfortable and kept "reassuring" me that it "wasn't necessary".

We're closing down our advance decisions charity, having (it seems) created false expectations about their efficacy. Feedback re. several ADRTs I supported people to write was that doctors failed to respect them.

Somehow it didn't seem a good idea to piss them off too much.

A nurse who observed all this + has LPA for her mum was horrified.
@drkathrynmannix @DyingMatters @StChrisHospice @gowerst_books Patients shouldn't need to educate doctors at the bedside. Patients shouldn't need to "tenderly invite" doctors to understand their EOL plans + help them with their emotional angst at respecting the law.
@drkathrynmannix @DyingMatters @StChrisHospice @gowerst_books The problem with the relentless focus on how to help patients articulate what they want is that when we follow the guidance, and raise issues early + have the correct forms properly completed, AND IT DOESN'T WORK, we feel helpless + as tho we've done something wrong.
@DyingMatters @drkathrynmannix @StChrisHospice @gowerst_books I had to do a lot of "emotion work" minutes before surgery to calm THEIR anxieties that I was casting aspersions on their medical skills, or that they'd have an (unnecessary) mortality - all of which could have been avoided if they'd been willing to have the convo earlier.

I just wanted them to recognise that I'd made autonomous decisions, and thought about them a lot in advance - and they were LEGALLY BINDING. They seemed entirely unprepared for this. Added hugely to the stress of admission + surgery.

For all the talk of research prioritisation, funders seem content to pump money into projects that add more to the pile of reports than to the sum of knowledge. Do HCPs understand that not using a patient’s unique identifiers alone doesn’t automatically make something OK to post online? Some stories are so unique they should never be posted, and if a patient or their family could identify them from the story, that isn’t okay to share
