@MarceloHelguera I've seen this happen when doing an Impella assisted VT ablation. The LV appeared completely collapsed and RV became dilated, but there was just enough flow for Impella to still provide support.

Thanks to @seth_j_worley bivtips crossed with KA2, glidewire, then amplatz superstiff, venoplasty, new RV and LV lead. Followed the steps as described in video.

Need advice. Pt in chronic Afib. Bradycardia with HR in sometimes 30-40s on beta-blocker for cad. Cardiology referred for pacemaker. They want him to stay on beta-blocker. Symptoms: some fatigue but can do whatever he wants to do. Reasonable to do pacemaker?

90 yo with paroxysmal afib on Flecainide 100 mg bid presented with afib rvr. Converted spontaneously. Followed by symptomatic sinus pauses. Flecainide effect? Or underlying sick sinus syndrome? Requires pacemaker? @KanwarAulakh@Firaszahwe @narrowQRS @KrishKancharla @drpchacko
