Most important reason for such 0.1 decimal changes in biochemical parameters are not because of patient faults...most likely because of Intra and inter lab error....there will be that margin of error for which no doctor will give any importance... We worry about 5.9 going to 6.9

Look how far we have come from the days of "MPGN & primary MGN" having advanced greatly through understanding the various pathophysiologies and IF staining, "FSGS" still a problem, time to ignore the scar and think of it in terms of podocytopathy or not, genetic or not.

@drtimothyli I pulled the scoring systems for IE based on organisms into a doc: docs.google.com/document/d/155… If you scroll down, there are 2 sources for Strep. And HANDOC has a few issues - A limited number of IE cases used - Not including all the different Strep spp - When TTE/TOE - unclear

@KIReports @GlomCon The evidence based answer to this question is- when the patient is at high or very high risk of progression to ESKD and has failed to achieve a consistent lowering of proteinuria to < 1.0gm/d with 3-6 months of optimum ACEi or ARB therapy ( ? combined with SGLT2i)
