That is to say a continuous loop of learning from each other in patient/family circles, in clinician circles and in mixed circles abt what happened and how to do caregiver care better. I guess I'm talking about a community of practice w robust pathways for comms.
Well, we know that when patients are discharged from the hospital quicker and sicker, family caregivers have to do medical care tasks often without being asked if they are willing or capable! #essentialfamilycaregivers were already stressed in 2018! Now more complex care!
It is frustrating to be told to discharge patients more quickly—as though we are inefficient and hold patients in hospital longer than necessary—by leaders and administrators who have no clinical background.
